Diego Alcazar is the son of Lorenzo Alcazar and Maria Sanchez. Diego grew up not knowing who his parents. It wasn't until after he come to Port Charles in search of his father that he learned his truth parentage. Diego had grown up believing that Maria was his sister.
After learning that Lorenzo was his father, Diego began following in his father's footsteps. Diego getting into the mobster lifestyle and Diego taking a liking to Mariah Maxmilliana "Maxie" Jones caused his relationship with girlfriend Brook Lynn Ashton to split.
After his relationship with Brook Lynn, Diego turned to his darkside. He became the Port Charles University stalker and started drugging girls and taking pictures of them, including Maxie and Brook. He claims he was getting revenge for the death of his cousin, Sage Alcazar, who was locked in a freezer by Brook and Georgianna "Georgie" Jones.
Diego went to prison for a few months but after he got out Diego developed a relationships with Georgie, who at the time was fighting with her husband Dillon Quartermaine. Diego teamed up with Lesley Lu "Lulu" Spencer, who had a crush on Dillon, to break up Dillon and Georgie. The plan works and Diego and Georgie start dating until they realize what Diego and Lulu had done. Diego told Georgie he accepted her for who she was and loved her and refused to give up on them which touched Georgie. But the damage was done and Georgie went back to her husband.
Diego was thought to have been killed after being shot in self-defence by Samantha "Sam" McCall. About a year and half later, Diego returned to town as Text Message Killer. He killed Leticia Juarez, Emily Quartermaine, Georgie Jones and Cooper "Coop" Barrett before he accidently hang himself during a fight with Nikolas Cassadine.
